Surgical Treatment of Aortic Pathology

Surgery for aortic pathology is among the most complex and risky procedures in the field of cardiac surgery. Within this realm, the most challenging cases involve patients with acute rupture and dissection of the aorta. Our institute has the largest experience in Ukraine, performing around a hundred surgeries annually for such cases. Our team of specialists has implemented all types of surgical interventions used in leading clinics across Europe and North America to provide the best possible care for patients with aortic pathology.

Furthermore, we take pride in our extensive experience in correcting post-coarctation aneurysms using the “ascending-descending aorta” bypass technique, which has been performed on 154 patients—a feat that few other institutions can match globally.

Our institute remains at the forefront of treating aneurysms of the thoracic aorta, having pioneered this field in Ukraine. According to the Association of Cardiovascular Surgeons, over two-thirds of all operations in Ukraine for ascending, arch, and descending thoracic aortic pathologies are conducted by our department. Despite the complexity of these procedures, our hospital’s mortality rate has been impressively low, with only 0.9% reported in 2016 (2 out of 223 operated patients).

Our dedicated team of doctors continues to innovate in the surgical treatment of various conditions, such as bicuspid aortic valve, complex forms of coarctation of the aorta, and other complex acquired heart defects. We specialize in procedures like multivalve prosthetics and valve-preserving operations for Marfan syndrome.

We constantly strive to improve our methods for correcting exfoliating aneurysms of the thoracic aorta, including advancements in deep hypothermia and retrograde cerebral perfusion techniques. These developments enable us to perform surgical interventions for aortic arch aneurysm correction lasting 120 minutes or more. Since 1980, we have successfully operated on nearly 1,000 patients with type A aortic dissection, with more than half of them receiving aortic arch aneurysm repairs.

Another notable achievement of our department’s scientific endeavors is the implementation of the latest world-class technologies, such as the simultaneous prosthetics of the ascending, arch, vessels of the arch, and descending thoracic aorta, known as the “Frozen Elephant Trunk” procedure.

Moreover, we collaborate closely with endovascular specialists in initiating and advancing hybrid surgery of the aorta. Our collective experience includes 89 operations, including complex vascular switches performed preoperatively.

In addition to our cardiosurgical techniques, we have widely adopted endovascular methods for treating acquired defects of the heart and major vessels. Patients with aortic dissection or aneurysm can benefit from endovascular stent-graft procedures (TEVAR) that are minimally invasive, involving the insertion of a stent-graft through a puncture in the common femoral artery, without the need for a chest incision.

Aneurysms in the abdominal part of the aorta are characterized by a permanent pathological dilatation of the vessel, where the diameter exceeds 1.5 times the expected diameter of the corresponding segment, taking into account the patient’s gender and body size. More than 90% of these aneurysms occur distal to the renal arteries. When the diameter exceeds 55 mm, the risk of rupture escalates to 5-10% per year. To address this condition, we offer endovascular replacement of the infrarenal part of the abdominal aorta (EVAR procedure), which follows the same principle as endoprosthetics of the thoracic aorta.
